Policy & Procedure for Setting Up Payment Plan Amounts
Company policy for arrangement of the payment amount will be based on the patient’s account balance due and will be broken down in tiers as follows:
Balances under $100 – paid in two months
Balances of $101 - $999 – paid in six months
Balances over $1000 – 10% of balance each month
We also take VISA, MasterCard, HSA in the mail by writing information on the back of your payment stub or you can call 360.424-6161
