In order to save you time during the check in process, we are working on putting together some of the forms that you need to fill out prior to your exam on this page.
You can either;
~ click on the link to open the document, fill it out on your computer, print & bring it with you to your appointment, **not all of the forms are able to be filled out online yet**
~ open the document, print it and fill it out by hand or
~ just wait to do it when you get to your appointment at the facility of your choice.
It’s all up to you!
Request your image CD or reports
Sometimes you feel you need to have a second opinion before further treatment. If you are going to a facility that is outside of Skagit County you will need to have your image(s) and report(s) sent to the facility of your choice, or you can pick them up and bring them with you to your appointment. If the facility that you are going to for the second opinion is within Skagit County there is no need to order your image(s) or report(s); most Doctor's offices have access to our PACS system which allows them to view the image(s) and report(s) on their computer.
Your exam images and reports are kept at the facility that you had your exam. Your medical record is maintained in a strict, confidential manner as required by state and federal laws. We will not release information contained in your medical record without your written authorization or as permitted by law.
You can request a copy of your report, images and find any pre-appointment paperwork by first choosing the facility below.
